How to Get Started:

1. Secure a Customer Information Sheet. Download from our website,
   OR Call 1-800-327-6012
   OR write MBA at 10955 Blackhawk Drive, Blue Mounds, WI 53517.
2. Fill out the Customer Information Sheet as completely as you can.
3. Return the completed form along with a map of your farm (FSA maps, available from your county agent are preferred. Other maps may be acceptable.) and any forage/tissue tests if you have them. Enclose payment per fee schedule below.
   (Send credit card number or check. You may also call us with your credit card number if you prefer not to send the numbers through the mail.)
4. You will in return receive from us a soil sampling kit with sample bags, instructions for collecting samples, and a submittal form to be returned with the sample to the lab we recommend.
5. Send your soil samples and submittal form to the lab. (Please do not send them to MBA.)  Due to the differences in soil labs and testing procedures, we do require that samples be sent to our recommended lab.
6. Once we have your Customer Information Sheet and the soil analysis back from the lab, we’ll put together a ‘soil story’ packet including test results, analysis of those results, and our recommendations. We’ll mail this to you in printed form. You will also be provided with the name and phone number of the consultant who prepared the recommendations. Call the consultant to set a phone appointment to discuss the recommendations and any questions you may have. 

    Our fees for sample analysis and phone consultations are based on the number of soil samples submitted:

Package A:  Up to 10 samples, Minimum $200 fee for up to 3 soil samples plus $50 per additional sample up to 10; includes one hour of phone consulting

Package B:  For 11-20 samples: Minimum $200 fee on first 3 samples plus $50 per additional sample; includes two hours of phone consulting

Package C:  For 21 or more samples: Minimum $200 fee on first 3 samples plus $50 per additional sample; includes three hours of consulting

More than 30 samples, call for special package pricing.

Additional phone consulting with any package:  $100 per hour.

On-farm consulting, including dairy/livestock, may be available in some areas; Call for cost estimate.
